  • Completed Business Account Application form
  • $5 minimum deposit to open a Business Share Savings account (required for membership)
  • $100 minimum deposit if you want to open a Business Checking account
  • Additional documentation for your business type, listed below
Minnesota:
  • Articles of Incorporation (filed with the MN Secretary of State)
  • Certificate of Incorporation (filed with MN Secretary of State)
  • EIN Verification (IRS Letter, signed W9, or a most recent tax return)
  • Certificate of Assumed Name, if applicable (filed with the MN Secretary of State)
  • Operating Agreement*
  • Beneficial Ownership Certification Form (NCUA Required)

 

Wisconsin:
  • Articles of Incorporation (filed with the WI Department of Financial Institutions)
  • Certificate of Existence (filed with WI Department of Financial Institutions)
  • EIN Verification (IRS Letter, signed W9, or a most recent tax return)
  • Certificate of Trademark, if applicable (filed with WI Department of Financial Institutions)
  • Operating Agreement*
  • Beneficial Ownership Certification Form (NCUA Required)
